Applying Lesen Premis (Business) / Store & Signboard License at Selayang (MPS)

Our service charge for :-

1. Applying Lesen Premise (Business) / Store = RM 250.00 - RM 350.00
2. Applying Lesen Iklan (Signboard) = RM 250.00 - RM 550.00

* NOTE : The above service charge are EXCLUSIVE of the license fees to MPS upon approval of the license.

The documents which we required are :-

1. Copy of Company Registration (SSM) - full set (3 sheets) or Copy of Memorandum & Articles (M & A) - full set, Form 24 & 49
2. Copy of Tenancy / Sales & Purchase Agreement - full set
3. Copy of I/C of one director / partner
4. Two (2) pcs of passport-sized photo of the same director / partner
5. Premise Layout Plan - you may get from the renovation contractor
6. Visual of the signboard - you may email to our address at ykh_rb@yahoo.com
7. Copy of Cukai Pintu & Cukai Tanah of current year - you may get from the landlord
8. Copy of Certificate of Fitness (CF) - you may get from the landlord
9. Copy of Bomba Cerfiticate for fire extinguishers - if you haven't bought any, we can recommend to our agent
10. Copy of the Injection Certificate for at least three (3) workers - only for rest. & cafe license
11. Copy of the Food Course Certificate for at lease three (3) workers - only for rest. & cafe license
