Applying Lesen Premis (Business) / Store & Signboard License at MPAJ (Ampang Jaya)

Our service charge for :

1. Applying Lesen Premis (Business) / Store = RM 250.00 - 350.00
2. Applying Lesen Iklan (Signboard) = RM 250.00 - 550.00

(* NOTE : The above service charges are EXCLUSIVE of the license fees to MPAJ upon approval of the license.)

The documents which we required are :-

1. Copy of Company Registration (SSM) - full set (3 sheets) or Copy of Memorandum & Articles of Association (M & A) - full book, Form 24 & 49
2. Copy of Tenancy / Sales & Purchase Agreement - full set
3. Copy of I/C of one partner / Director
4. 2 pcs of passport-sized photo of same partner / Director
5. Copy of Cukai Pintu for current year - you may get from your Landlord
6. Copy of Certificate of Fitness (CF) - you may get from your Landlord
7. Copy of Bomba Certificates for fire extinguishers - if you haven't bought any, we can recommed to our agent
8. Visual of the signboard - you may email to our address at ykh_rb@yahoo.com
9. Premise Layout Plan - you may get from the renovation contractor
10. Copy of Injection Certificate for at least 3 workers - only for restaurant & cafe license
11. Copy of Food Course Certificate for at least 3 same workers - only for rest. & cafe license
12. Copy of Injection Certificate for at least one person - for Beauty & Hair Saloon license
