The documents which we required are :-
- Copy of Company Registration (SSM) - full set (3 sheets) or copy of Memorandum & Articles of Association (M & A) - full book, Form 24 & 49
- Copy of Tenancy or Sales & Purchase Agreement (full set)
- Copy of I/C of one partner or Director
- Visual of the signboard - you may email to our address at ykh_rb@yahoo.com
- Premise Layout Plan - you may get from the renovation contractor
- Copy of Injection Certificate for at least 3 workers - only for restaurant & cafe license
- Copy of Food Course Certificate for at least 3 same workers - only for rest. & cafe license
- Copy of Beauty Certificate - only for Beauty & Hair saloon license
- Company Rubber Stamp (with co.name & premise address)
First of all, I'm Joseph from Sunway University. My group is doing a business start up project regarding F&B. I have a couple of questions, hopefully you can help up. I found some info in SSM website saying the registration fee is RM60 for partnership using trade name. i wonder if the amount u state above (RM250-350) has sth to do with the RM60 from SSM website. and the signboard licensing cost is it still applicable for now(2011)? where can i source for the licensing fee?
